Already registered for Patient Portal?
Sign Up for Patient Portal with an Email Invitation
K1. All new clients to ACTS will be asked for their email during the intake process and if email address is provided, a portal invitation will be sent automatically to the email that was provided. If you are a past client in need of records, email portalaccess@actsfl.org to request access to the Patient Portal.
2. Check your email for an invitation from CredibleInc. The invitation will expire in five (5) days. You will need to use the Registration Key that is provided in the email.

4. Click the link in the email to register for the portal. Enter the following information:

- Registration Key; First Name; Last Name; Birth Month; Birth Day
- Click Next

5. You will then be prompted to answer 3 security questions.
6. When finished, click OK. You will then be able to use your UserID and password to log into the portal.
